Individual plan 

    Adults (age 19+) = $89/month

    Children (age 18 & under) = $59/ month

Family Plan

    Up to 2 adults & their dependent children (age 25 & under) = $195/ month

Couple Plan

2 adults =  $165/month

ENROLLMENT FEE = one time $80 fee per new billing account (family/household)

This is for minor sick care and injuries. This option is great for those needing to access care quickly for minor illness without having to wait in an urgent care clinic or when your personal PCP is booked and you can't be seen acutely.
